Semaglutide — GLP-1 Acts on one pathway. A synthetic peptide that mimics the body's natural GLP-1 hormone — may help curb appetite and slow digestion, leading to potential weight loss.

Tirzepatide — GLP-1 + GIP Acts on two pathways. Dual receptor activity may improve blood sugar control, reduce appetite, enhance fullness, improve insulin sensitivity, and lead to potential weight loss.

Hormone Replacement Therapy (HRT)

Find relief from perimenopausal and menopausal symptoms

Included therapies:

SYSTEMIC HORMONE THERAPY - Estradiol — patch, pill, or gel Micronized progesteron

LOCAL / VAGINAL HORMONE THERAPY - Estradiol vaginal cream Estradiol vaginal inserts

NON-HORMONAL OPTIONS - Paroxetine Desvenlafaxine succinate ER

What is Systemic Estradiol -

Estradiol is a form of estrogen delivered throughout the body as a patch, pill, or gel. It is the program's first-line systemic hormone therapy for menopausal symptoms.

How it Works -

Systemic estradiol is FDA-labeled for vasomotor symptoms1, Two such as hot flashes and night sweats, for osteoporosis prevention, and for vulvar and vaginal atrophy symptoms such as vaginal dryness, irritation, and painful intercourse. Some patients also report improved sleep and mood, though this is not an FDA-labeled indication.

When vaginal/genitourinary symptoms are the only concern, topical vaginal products should be considered first.

ADMINISTRATION OPTIONS Patch — 0.025 mg (mild), 0.05 mg (moderate), 0.0750.1 mg (severe), twice weekly to lower abdomen or upper buttock

Pill — 0.5 mg daily (mild) up to 1 mg twice daily or 2 mg daily (severe)

Gel — 0.25 mg daily (mild) up to 11.25 mg daily (severe), applied to the thig

What is Vaginal Estradiol -

Low-dose estradiol delivered directly to vaginal tissue as a cream or insert. It treats the genitourinary symptoms of menopause and is absorbed locally rather than systemically.

HOW IT WORKS -

Estradiol increases moisture, blood flow, and elasticity in the vaginal, vulvar, and bladder tissues. The genitourinary changes that follow menopause are chronic and worsen over time if untreated, so vaginal hormones are generally recommended indefinitely.

ADMINISTRATION OPTIONS

Vaginal cream 0.03% 0.3 mg/mL — 0.15 mg 0.5 mL) nightly for 2 weeks, then 23 times weekly

Vaginal tablets / inserts — 6 mcg or 12 mcg nighty for 2 weeks, then 23 times weekly.

Primary Conditions and Use Cases:

Hot flashes and night sweats

Sleep disruption and fatigue

Mood changes, anxiety, depression

Brain fog and cognitive concerns

Vaginal dryness, painful sex, and bladder changes

Low sexual desire

Weight changes and joint pain

Skin and hair changes

What is Micronized Progesterone -

Micronized progesterone is an oral hormone taken at bedtime. It is required for women with a uterus who take systemic estrogen, in order to protect the uterine lining. H

HOW IT WORKS -

Estrogen alone thickens the lining of the uterus and increases the risk of endometrial cancer. Adding progesterone, or having a Mirena IUD, protects the lining and reduces this risk. For most patients, progesterone also helps induce sleep and reduce anxiety.

ADMINISTRATION OPTIONS

Oral — taken by mouth at bedtime

Continuous — 100200 mg nightly (common)

Cyclical — 200 mg nightly for 12 days of the month (not common)

What is Non-Hormonal Options -

Paroxetine and desvenlafaxine succinate ER are non-hormonal prescription pills used to reduce hot flashes and night sweats in patients who cannot or prefer not to use hormone therapy.

HOW IT WORKS -

Paroxetine (an SSRI) and desvenlafaxine (an SNRI) increase serotonin — and, for desvenlafaxine, norepinephrine — activity in the brain. This is thought to help stabilize the hypothalamic thermoregulatory center, which becomes hypersensitive to small temperature changes during the menopause transition, reducing the frequency and severity of hot flashes and night sweats.

ADMINISTRATION OPTIONS -

Paroxetine — 10 mg by mouth daily, with or without food; do not crush or chew Desvenlafaxine succinate ER  50100 mg by mouth daily 100 mg standard)
